Odyssey Middle Schools' health room is staffed by a School Health assistant. It is very important that every student has a completed and updated Emergency Form on file in the registration/attendance office for each school year.

Pursuant to Florida Statute 1001.42, HB 1557;a parent/guardian MUST opt In for health services every school year.

For Odyssey students to receive any services in the health room including 6 grade screening services such as hearing, vision, scoliosis, weight, and height. Medication only if provided by the parent/guardian. Parents must complete the online consent forms

If your child has a medical condition that may require emergency medication for in school use, such as; Asthma, seizures, anaphylaxis or any other health condition that may require a health plan please contact Ms. Cabrera our school health assistant.

Students are not to carry any medication, over-the-counter or prescribed, at any time during the school day. Except for headache medication regulated by the United States Food and Drug Administration as per the new House Bill 1537, Parents are encouraged to give medication at home specially the first dose of a new medication in case student develops an allergic reaction.

If prescribe medication MUST be given at school, the pharmacy label must state the time to be given and time must be within school hours.

The Health assistant will count and document the quantity of tablets being dropped off in front of the parent/guardian.

All medication left in the school's health room must be accompanied by a written authorization form from a parent/guardian. If medication is over the counter, it must be in its original container, sealed, not expired, age appropriate and handed to the health assistant or the back up only. Once medication has been received, both the parent/guardian and health room staff receiving the medication must sign a recipe form.

Any self carry medication such as Inhalers, Epi-Pen and diabetic pump/Glucometer must have a self carry form on file, filled out by a medical provider and returned to school health room.

It is imperative that the registration office be notified immediately in the event of a change of address, home or office telephone number, or if emergency pick up information changes during the academic school year.
